PHONE CONSULTATION SERVICE

Thank you for your interest in our phone consultation service. Consultations are available for out-of-state women who are not able to find a postpartum specialist in their local area. (Please note that these consultations are not diagnostic and medication questions will be referred back to your physician.) Please read the information below before completing the questionnaire.

Objectives of consultation:

1. Initial assessment of symptoms, history, risk factors, current level of functioning

2. Recommendations for treatment options

3. Suggestions for self-care and coping strategies

4. Support

5. The purpose of this assessment is not to substitute for a comprehensive evaluation. It should serve as an adjunct to your current treatment program or as an incentive to find the appropriate treatment.

Setting Up Your Phone Consultation:

1. Click on the "Consultation Form / Questionnaire" button to the left.

2. PRINT out the PPD Assessment Questionnaire.

3. Complete all questions. Answer as honestly as you can to insure the most accurate assessment.

4. Mail the completed PPD Assessment Questionnaire to:
   The Postpartum Stress Center

   Attention: Phone consultation

5. Fee must accompany questionnaire (checks only).

6. When we receive your completed questionnaire and payment, someone will contact you to set up the appointment. You agree to call The Postpartum Stress Center on the appointed day and time. If you cannot keep the appointment time, 24 hours notice will be required in order to receive credit for your prepaid charge.
